Segment Two - Contract Language
COURSE PROVISIONS

Life Drivers Training will provide a minimum of 6 hours of classroom instruction provided by a certified instructor. Classroom instruction shall not exceed 2 hours per day.
REQUIRED LANGUAGE

NOTICE- This provider is required to be certified by the Secretary of State. If you have any complaint, which you cannot settle with this provider, write: Michigan Department of State, Driver Programs Division, Lansing, MI 48918. Completion of driver education instruction does not guarantee qualification for a driver license.

For a student to participate in Segment 2, verification must be received that the student has completed a minimum of 30 hours of driving (including 2 hours at night) with a licensed parent or guardian (or parent designee) on a Level 1 license, which has been held for not less than 3 continuous months.
TERMS

Course Requirements: Earning a Segment 2 certificate from Life Drivers Training requires each student to attend and participate in all classroom sessions. Students must pass the Segment 2 State Test with a score of 70% or better, as well as complete all activities and assignments during the session.

Tuition: Segment 2 tuition is $75.00 and is due in full no later that the first day of class. Life Drivers Training will accept cash, check, money order or credit card for the full amount of tuition. A $25.00 fee will be charged for all returned checks.

In the event that any behind-the-wheel instruction is requested and scheduled between the student and/or parent and Life Drivers Training, this will be offered as a free service to the student. In the event that driving is scheduled, a special driving waiver must be signed by the parent and added to the students file.

Refund Policy: Life Drivers Training will not refund any fee, tuition, charges or any part thereof should we be ready, willing and able to fulfill our services as outlined herein. If a student is unable to complete a program, the tuition received may be credited toward an available future program of the student’s choice and with Life Drivers Training’s approval. If a student is removed from class for misbehavior, credit will not be given.

Life Drivers Training reserves the right to cancel, postpone or reschedule classes or lessons at our sole discretion.

Students need to arrive on time for all activities. Excessive tardiness will result in removal from class. Five (5) minutes late equals one (1) tardy; three (3) tardies and the student will be dismissed from class.

Students need to be prepared for class with pens, pencils and notebooks.

Let it be noted that there currently are no additional costs associated to supplies, books, workbooks, replacement certificates, etc. the amount due will be based only on tuition rates.

Make-up days: There will perhaps be days when students are absent, based on sickness or emergencies. The Student will be required to make up all missed classroom time, with the instructor currently instructing that
